Kinship supports the weeks after discharge with optional nurse visits at home, medicines checks and a Kinkeeper who checks every report.
Discharge is when the hard part starts. Medicines to take on time, wounds to watch, a follow-up appointment nobody has booked, and a relative who says they are fine because they do not want to worry you.
A visit rhythm for the first month or two gives you someone qualified in the room, and a Kinkeeper who reads every report and knows what a normal recovery looks like.
What Kinship does here
Kinship Visits, with dates agreed
Weekly visits provide regular support at home. Agree the visit type and activities with the person receiving support. Nurse visits are one example; treatment stays with their doctor and hospital. Updates are shared with permission.
A named Kinkeeper
A Kinkeeper comes with Kinship Visits: a named, desk-based registered nurse who reads every visit report and is who the family calls. Updates follow the person's consent and permissions. If a report shows something that should not wait, she says so.
Medicines and appointment reminders
Kemi checks in on WhatsApp and reminds them about medicines and appointments, passing worries to the Kinship team.
When something is wrong
Your desk-based Kinkeeper reviews visit reports and discusses concerns with the person and those they permit. In an emergency, contact local emergency services first. Treatment costs are separate.

Visits happen at home. The Kinkeeper reviews reports, with updates shared according to the person's consent and permissions.
What it costs
- Kinship Plan £20 a month when billed monthly: the Kinship care team, Kemi on WhatsApp and a record shared according to the person's consent and permissions.
- Kinship Visits add £40, £75 or £140 a month for 1, 2 or 4 visits per month when billed monthly.

Can this start quickly?
Tell us the discharge date when you get early access and we will tell you honestly how soon a first visit can happen in their area.
Can I reduce the visits once they are better?
Discuss a different visit rhythm with the person receiving support and the Kinship team. We will confirm the arrangements and availability.
Will the Kinkeeper talk to their doctor?
With the member's agreement, yes. The Kinkeeper can pass on what the visits have found and help arrange the next appointment.
Does the Kinship Plan pay hospital bills?
No. Hospital bills, medicines and tests are paid separately.
