Managing high blood pressure or diabetes for a relative in Nigeria, from abroad
High blood pressure and type 2 diabetes are managed by four boring things done consistently: regular readings, medicines taken as prescribed and reviewed on time, a diet the person can keep to, and somebody who notices when the numbers drift. From abroad the last one is the hard part.

How common they are
About a third of Nigerian adults have high blood pressure, rising to more than half of people over 50 across Africa, and the South-West has the highest rate in the country. About 7% of Nigerian adults have type 2 diabetes, up from about 2% in the early 1990s. Both are usually silent until they are not, which is why a relative who says they feel fine can still be a month from a stroke.
Why they go wrong at a distance
- Medicines run out and are not replaced, or are replaced with a cheaper alternative from a chemist.
- Readings are not taken because nobody asked, or a monitor was never bought.
- Reviews are missed because the appointment costs money and the person feels fine.
- Symptoms are downplayed on the phone so as not to worry anyone.
The set-up that works
- A home monitor (blood pressure cuff, glucometer) and a simple log. A photo of the log every week is enough.
- A standing order for medicines, paid for in advance, so there is never a gap.
- A review date in the calendar, with someone to go with them.
- One person, ideally qualified, who sees the readings and knows what a drift looks like.
- An agreed rule for what counts as urgent, written down, so nobody has to decide at midnight.
